With 685 beds, we are home to the only comprehensive Burn and Level I Trauma center between Los Angeles and Sacramento. The 56,000 square-foot emergency department is one of the largest and busiest in California! We have an 84-bed Level III NICU and plan to significantly expand pediatric specialty care. Our hospital serves 2.5 million people within 9 counties, averaging a daily census of 641 patients. We're affiliated with UCSF as a teaching facility; trained 3,000 Medical Doctor's to date plus continuous invests in research and training grants.

Responsibilities: The Admissions Coordinator plays a crucial role in facilitating the admissions process, ensuring a smooth transition for new residents and their families. The Admissions Coordinator receives admission/discharge/transfer information from internal and external sources and facilitates resident placement in the appropriate bed/unit. Conducts tours and pre-admission interviews with prospective residents/responsible parties and explains all facility services. Collaborates with the multidisciplinary team on discharge processes and serves as a liaison for community facilities and resources. Accountabilities 1. Census Development: Lead census meetings and anticipate bed movement and upcoming discharges. Responsible for coordinating the process of admitting new residents.2. Referral Management: Serve as the first point of contact for inquiries and referrals, manage incoming referrals, and obtain necessary clinical and financial information.3. Screening and Assessment: Evaluate/Screen (Ensocare or directly with Case Management) potential residents based on admission criteria, including medical and social needs as well as bed availability. Coordinate paperwork, collaborate with various departments, and provide support to new residents and their families.4. Intake and Documentation: Complete necessary admission paperwork, including insurance verification (e.g., obtaining authorizations) and financial arrangements (e.g., answering questions regarding financial responsibility, insurances, and Medicare/MediCal eligibility).5. Communication and Coordination: Liaise with healthcare professionals, social workers, and families to ensure a comprehensive understanding of the resident's needs. Communicate with hospital and facility discharge planners, and partner with the DON to determine appropriate levels of care and placement.6. Customer Service: Provide excellent customer service, answer inquiries, address concerns, and build positive relationships with potential residents, families, and referral sources.7. Interdisciplinary Team Collaboration: Collaborate with the nursing staff, Social Services, Therapy, and Business Office to ensure timely and appropriate admissions and a smooth transition.8. Admission Process: Provide tours of the facility and explain its policies and procedures to new residents and their families. Foster a positive first impression for residents and families.9. Ongoing Support: Provide support to new residents and their families and address any concerns or questions to ensure a smooth and efficient transition.10. Perform other job-related duties as assigned. Qualifications: Education• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required• Bachelor's Degree in a healthcare related field preferred Experience• 3 years of experience in a skilled nursing facility or healthcare setting required Disclaimers:
